What is chronic care management?

Chronic care management is a Medicare program that provides extra medical care and monitoring services to people with at least two chronic conditions. You can enroll in Medicare three months before you turn 65.

Chronic care management isn’t automatic if you have qualifying medical conditions. AVC Med can recommend your enrollment to Medicare, allowing access to a broader range of health care. 

What conditions are included in chronic care management?

Chronic care management covers many diseases, conditions, and health issues. Some of the most common among them are: 

• Asthma
• Arthritis
• Diabetes
• High blood pressure (hypertension)
• Cancer
• Heart disease
• Depression
• Alzheimer’s disease and other dementias
• Neurological diseases
• Autoimmune diseases

Many other conditions may qualify for the chronic care management program as well. Medicare considers conditions chronic if they’re expected to last a minimum of 12 months. 

What does chronic care management include?

Chronic care management includes both in-person medical care and telemedicine. Your first visit (the initiating visit) is a face-to-face appointment with an experienced AVC Med provider who comes to your home (house call). 

During your initiating visit, you’ll likely have various screening tests to evaluate your blood sugar level, cholesterol level, and other important aspects of your health. You may need a chest X-ray or other radiology tests as well. AVC Med provides all these things through their mobile medical solutions vehicle that comes directly to your doorstep.

Your chronic care management includes personalized health support and guidance to achieve a healthier lifestyle. This reduces your risk of additional chronic diseases while preventing your current diseases from worsening. 

Most chronic care management visits occur through telemedicine, in which you’ll meet your medical provider through videoconferencing on your smartphone, computer, or tablet.  During telemedicine visits, you can see a medical care provider with experience in chronic disease management.

You’ll receive medical care focused on your diseases and other health issues that may arise. Your provider can monitor your blood sugar, cholesterol, blood pressure, and other health metrics remotely in many cases. Chronic care management can help with healthy living, disease management, illness prevention, and whole-body health and wellness.
